Meaghan O’Neil has over 20 years of experience in designing and improving mission critical and safety critical systems. She currently provides consulting in innovation, transformation and systems engineering as the founder of System Design and Strategy LLC. Meaghan was co-author of the System Safety section for version 5 of the INCOSE Handbook. Meaghan is a founding co-chair of the INCOSE System Safety Working Group

Professor Duncan Kemp is the Senior Fellow for Systems Engineering (SE) in the UK Ministry of Defence. Duncan has experience in delivering safe and effective systems in Defence, Information Services, and Rail. He is both the discipline lead for SE and the team leader of the MOD’s 70-person internal SE consultancy. Duncan is a chartered professional engineer, Fellow of the Institution of Engineering and Technology and INCOSE Fellow. Duncan was the lead author of the System Safety section for version 5 of the INCOSE Handbook and is currently the co-chair of the INCOSE System Safety Working Group. He is the Visiting Professor for Systems Thinking at Loughborough University.
